This quest becomes trivial above Level 104.

Find the Spirit Altars and bring me 3  Spirit Skulls.

Spoiler Alert! We have some more information about how to perform this quest but recognize you might want to try it on your own before we tell you the secret...

There are more than one Spirit Altars in the area, but only one is marked on the map. Each Spirit Altar is surrounded by 3 Forsaken Skeleton Mages. They are too busy doing whatever they are doing and will ignore you so long as you do not hit them. Unfortunately, the Gruesome Talisman is only revealed when you kill all 3 of them. If you are quick you can gather more than one  Spirit Skull from a single Gruesome Talisman.
