To find this mob in-game, use BACKSPACE to open the World Search window, then enter Kaiz Ballusha in the Search field and click "Search". Remember, the field is case-sensitive and must be entered exactly as shown.
This mob is a member of the group collectively known as Black Codex.
Rune Exchange Interface
This NPC is the only Rune Exchange Merchant in the game. They never added any more after this patch.

Ancient Mementos are dropped from Dungeon Bosses in Heart of the Ocean and higher dungeons.
