There is an underwater "back door" to the Altar of the Dying Sea.

Go south from Citadel Outpost and to the right. See the rope bridge? On the right is a ledge-cavern with a stairway up. At the top will be 2 Scealoks of the Dying Sea but they are easily handled. Then, fight your way to the rope bridge and use it to enter Altar of the Dying Sea.
